29 CFR 1926.20(b)(2):     Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    Employees working from a four in twelve pitch roof were exposed to falls of approximately 11 feet to a lower level. The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.

29 CFR 1926.102(a)(1):     Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    Employees installing shingles with a pneumatic nail gun were not wearing ANSI Z 87.1 rated eye protection. The employer failed to provide his employees with eye protection while using a pneumatic nail gun.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    Employees were engaged in roofing activities working from a 4 in 12 pitch roof, employees were exposed to falls of approximately 11 feet from the ground to the eave. Employees were not protected from falling by guardrail systems, safety net systems, or a personal fall arrest systems.
