29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    a)  Second Floor Ledge - A laborer installing Tyvek wrap around a newly installed window was not provided with fall protection and exposed to a 14 ft. fall to the earth below.

29 CFR 1926.404(b)(1)(iii):  Where ground-fault circuit interrupters were not utilized, employer did not establish and implement an assured equipment grounding conductor program on the construction site covering all cord sets, receptacles which are not part of the building or structures, and equipment connected by cord and plug which are available for use or used by employees:     a) Front Yard - A wet saw used to cut ceramic tile and which was provided temporary power was not provided with ground fault circuit interrupter circuitry or and assured equipment grounding conductor program.    b)  Back Yard - A Makita miter saw used for cutting wood and which was provided temporary power was not provided with ground fault circuit interrupter circuitry or and assured equipment grounding conductor program.
