29 CFR 1926.351(b)(4): Welding cable(s) in use were in need of repair:    On or about September 04, 2012, employees performing welding activities used cables and cords that had cuts and tears with exposed internal wires and wires pulled out of the protective sheathing.  Employees were exposed to electrical shock hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about September 04, 2012, employees performing welding activities, installing steel beams and metal roofing material on the front edge of the roof, were not protected from falling approximately 22 feet to the ground below.
