29 CFR 1918.33(a): Employees were permitted to pass fore and aft over or around the deck loads:    Port of Anchorage, Barge "Kuskokwim Trader," Port Rail:  On or about September 3, 2012 and at times prior therto, during marine cargo handling operations, an employee was allowed to "walk" lashing chains in a location where the forklift operator could not observe the employee.  This condition exposed the employee to a caught-between hazard resulting in trauma requiring hospitalization.      NORTHLAND SERVICES, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 307505172, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON 10/29/2009, WITH RESPECT TO A WORKPLACE LOCATED AT THE BARGE "HUNTER BAY" IN THORNE BAY, ALASKA.
