29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:    Bernal, Alejando at 7015 Shady Bend Drive., Fort Collins, CO 80225:  On and before August 9, 2012,  the employer did not ensure that employees were provided eye protection when power tools such as pneumatic nail guns were used during framing operations.  Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    (a)	Bernal, Alejando at 7015 Shady Bend Drive., Fort Collins, CO 80225:  On and before August 9, 2012 the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities on the roof of a residential home had exposure to a fall hazard from unprotected roof edges.  This condition exposed the employees to the hazards of a fall of approximately 18-20 feet.    (b)	Bernal, Alejando at 7015 Shady Bend Drive., Fort Collins, CO 80225:  On and before August 9, 2012 the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities inside on the 1st level of a residential home had exposure to a fall hazard from a floor opening that lead into the basement.  This condition exposed the employees to the hazards of a fall of 8 feet 10 inches.
