29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a)Building T-9166 (Fort Campbell) - On or about 8/28/2012, employees were exposed to electrical hazards when working with a portable paint sprayer that was missing the ground prong on flexible cord.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: (Construction Reference 1926.103)    (a)Building T-9166 (Fort Campbell) - On or about 8/28/2012, employees were exposed to respiratory hazards and were using respirators voluntarily.  The employer did not establish and implement those elements for voluntary use of a respirator in the written program.
