This violation occurred on September 11, 2012, at a job site located at 6926 Military Parkway, Dallas, Texas 75227.    a)  At least four (4) employees were working at building A on the roof installing a new roof and were not tied off to protect them from falling to ground level below.    b)  At least two (2) employees were working at building B on the roof installing a new roof and were not tied off to protect them from falling to the ground level below.
