29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 09/11/12 at Byron, GA:  Employees were exposed caught between hazards while performing work adjacent to unguarded conveyor belt ends at a concrete mix plant.  The employer failed to properly guard all ingoing nip points at the following locations:  a. Convyor System - Gravel:  The employer failed to properly guard the lower end of a conveyor system, which fed gravel into the hopper/bin at the mixing plant.  b. Conveyor System - Load Belt: The employer failed to properly guard the lower end of the load belt. Several holes were cut into the guard in order that maintenance could lubricate the conveyor system.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    On or about 09/11/12 at Byron, GA: Employees were exposed to electrical hazards while working at the cement mixing plant.  The employer failed to provide a cover on a electrical panel, which was used in the chemical mixing process.
