29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a. On or about September 7, 2012, each employee engaged in installing roofing shingles on a 4:12 pitch roof were not protected from falling 11 feet to the ground by the use of a guardrail system, safety net system, personal fall arrrest system or any other type fall protection method.     Abatement Certification and Documentation are not required

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    a. On or about September 7, 2012, the ladder placed in front of the doorway to the home was not extended 3 feet past the landing and not secured to prevent accidential movement.    Abatement Certification and Documentation are not required
