29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The employer failed to develop energy control procedures and provide training to employees. Control of hazardous energy procedures should include but should not be limited to the following equipment: Okazaki Laminator, Slitters, Totani Pouch Making Machines, Hensen and Totani Spout Inserters. No Abatement documentation is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1- The employer did not use adequate machine guarding to cover the blades, conveyor belts and rollers on the Green Totani. Employees were exposed to cuts, lacerations, fractures and amputations. 2- Employees were exposed to cuts, lacerations and amputations while removing the blades, replacing the blades and removing excess pouch material from production machines. This includes but is not limited to the pouch making machines HT-08, HT-07, HT-32, HT-33 and HSI04 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The first set of gears was 32 inches from the ground and the second set of exposed gears was about 67.5 inches from the ground.
