29 CFR 1926.451(g)(4)(ii): The top edge height on all suspended scaffolds where both a guardrail and a personal fall arrest system were not between 36 inches (0.9 m) and 45 inches (1.2 m) above the platform surface:  Note:  For supported scaffolds manufactured and placed in service before January 1, 2000, and on all suspended scaffolds where both a guardrail and a personal fall arrest system are required, the top edge height should be between 36 inches (0.9 m) and 45    a) Employees were exposed to fall hazards of approximately 33 feet 8 inches to the ground below while working from fabricated frame outriggers that had either no endrails or endrails installed at approximately 32 inches from the top of the rail to the scaffold platform.     ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Employees were exposed to fall hazards of 30 feet 8 inches to the ground below while working within 1 foot of the unprotected edge of the 4th floor with no fall protection    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
