29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Peak West Corporation @ 12875 E. 42nd Ave., Denver, CO 80239: On 7/19/12, the employer did not ensure that employees were protected from a fall hazard. One employee was exposed to a fall hazard when he cut 2, L-shaped, holes in the roof, each of which measured approximately 3.9sqft. The employee was not protected from the fall hazard by a guardrail system, safety netting, personal fall arrest system or otherwise. This hazard exposed the employee to an approximate fall hazard of 30ft.
