29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) While working on top of the Bag House, employees are not protected from falling by the use of standard guardrails or the used a personal fall protection system, fall hazard of 20 feet. b) While accessing the Bag House, employees are raised by the Clark Forklift, with a pallet on the forks. The pallet does not have guard rails and no personal protection is used. c) West side of building, the Cat Walk next to the dryer. The Catwalk has a top rail at 33 inches high but no mid-rail. d) On top of the Dryer, no fall protection is provided.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. In the Shop Area, north bench, on the Buffalo, 6 inch electrical bench grinder, the faceplate on the110 volt electrical switch was missing, exposing the inner electrical connections.
