29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder:    This violation occurred on September 18, 2012, at a job site located at 2404 Mallory, Lancaster, Texas 75134; where at least six (6) employees used an extension ladder that did not extend 3-ft above the upper landing to access/exit the roof.
