29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:    a) Chelsea at East Brunswick/East Brunswick, NJ Facility: The employer did not have a written certification of the hazard assessment for employees who were exposed to chemical hazards while handling (i.e. transporting, pouring, spraying, etc.) chemicals such as, but not limited to:    - sodium hypochlorite (contained in StarBrite, having pH of 12.6),    - isopropyl alcohol, 2-butoxyethanol (contained in BriteBlue, having pH of 11.0-12.0),   - phosphoric acid and muriatic acid (contained in Neutra Sour, having pH of 1.8),  - n-alkyl dimethyl benzyl ammonium chloride, dodecyl dimethyl ammonium chloride (contained in Lemon Hi-Con 64, having pH of 6.0-8.0). The employer did not have each affected employee wear appriopriate protection for eyes and hands to protect them from chemical hazards.    Condition observed on/or about September 20, 2012.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.    a) Chelsea at East Brunswick/East Brunswick, NJ Facility: The employer did not provide chemical resistant gloves which were appropriate for use to protect employees from chemical burns while using corrosive chemicals.  1) Employees working in the Laundry room are exposed to chemicals such as but not limited to:    - sodium hypochlorite (contained in StarBrite, having pH of 12.6),    - isopropyl alcohol, 2-butoxyethanol (contained in BriteBlue, having pH of 11.0-12.0).   - phosphoric acid and muriatic acid (contained in Neutra Sour, having pH of 1.8).  2) Employees working in the housekeeping department are cleaning areas such as bathrooms and are exposed to:  - n-alkyl dimethyl benzyl ammonium chloride, dodecyl dimethyl ammonium chloride (contained in Lemon Hi-Con 64, having pH of 6.0-8.0).    Condition observed on/or about September 20, 2012.
