29 CFR 1926.1053(b)(13):  The top or top step of a stepladder shall not be used as a step.    On or about April 25, 2012, an employee performing framing activities used the top step of a 6 foot step ladder.  Employee was exposed to a fall of approximately 6 feet to the concrete below.
