29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 9/17/12    Location: 365 Central Avenue, Scarsdale NY 10583    a) A 24 foot extension ladder used to access a flat roof at a height of  11 feet did not extend 3 feet above the landing level. The ladder extended extended 1 foot above the landing level.
