(a) Worksite, equipment yard - Employee working on the an elevated platform without standard railings was exposed to falling 11 feet to the ground below, on or about 9/20/12.      NOTE: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.180(e)(2)(iii)(a): After repairs and alterations had been made, the crane was not tested in accordance with SAE Recommended Practice, Crane Load Stability Test Code J765 (April 1961).    (a) Worksite, equipment yard - The truck mounted, National Crane Corp. Knuckle Boom Crane, Model # N-45-29, Serial # 11833 used to hoist a dump truck cargo cover was not tested to determine the maximum capacity of a crane to counterbalance loads applied to its hook block. The capacity of the crane is to be reported in terms of the load in pounds and its corresponding radius in feet for a specified position of the superstructure with respect to the mounting, on or about 9/20/12.    NOTE: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
