29 CFR 1926.451(g)(1):   Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) Construction site, side of pavillion - On or about August 8, 2012 employees are doing carpentry work from a stack of wood sheathing, supported by a Pettibone model B-68 forklift at a height over 10 feet, without being protected from falling.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Construction site, roof of Pavillion - On or about August 8, 2012, employees are installing wood sheathing more than 10 feet above grade without using any form of fall protection.
