29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by flying objects:  a. Machine Shop - On or about September 5, 2012 an employee experienced multiple injuries while pressing an insert onto a die with an inadequately guarded hydraulic power press (MS-DA3).  Employees were thereby exposed to struck by hazards.
