29 CFR 1926.404(b)(1)(iii)(C):     The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage:    a)  The employer does not ensure that flexible cord sets are inspected for damage and, if damaged, removed from service.  This violation occurred on or about September 20, 2012, in the site preparation area, where employees were using a Dewalt circular saw connected to a flexible cord set that was missing a portion of the outer insulation, revealing the inner insulated conductors.    b) The employer does not ensure that flexible cord sets are inspected for damage and, if damaged, removed from service.  This violation occurred on or about September 20, 2012, in the site preparation area, where employees were using a Makita circular saw (Model 5007F) connected to a flexible cord set that showed the visual effects of internal damage from use under an overcurrent condition.
