29 CFR 1910.134(g)(1)(i)(A):     Respirator with a tight-fitting facepiece was worn by an employee who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function.    a) Jobsite:  The employer allowed employee to maintain facial hair while wearing a half mask respirator which inhibits  a proper face to mask seal, potentially exposing the employee to respiratory hazards associated with the removal of brick mortar.

Jobsite: The employer did not maintain copies of Material Safety Data Sheets for hazardous chemicals including crystalline silica.   Employees were exposed to respirable crystalline silica from the grinding  of brick mortar, and the cleanup of the resulting debris.
