29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) On or about August 30, 2012, employees working on a multi-pitch roof did not have a personal fall arrest system to protect them from a 35-40 foot fall to ground level.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder. a) On or about August 30, 2012; front of home; employees were using an extension ladder to access the roof that was not properly secured exposing the emplyeees to a 25 foot fall if it is displaced while accessing it.
