29 CFR 1910.37(b)(1):  Each exit route was not adequately lighted so that employees with normal vision could see along the exit route:      a) 201 Leverington Ave., Production Areas, and Stair Tower - Numerous emergency lighting fixtures throughout the facility were found to be non-operational.  These fixtures had been installed at various locations throughout the four story building but, when tested on 9/25/2012, did not operate.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees were informed of this abatement.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of the radial saw was not guarded to the full diameter of the blade by a device that shall automatically adjust itself to the thickness of the stock and remain in contact with the material being cut:     Shipping Area, Delta Radial Saw (Model 33-527) - Employees were exposed to an amputation hazard where the lower portion of the blade for the Delta saw was not guarded.  Observed 9/25/2012.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:      a) Machine Shop, Metal Working Lathes (South Bend & Fortune S2060)  Employees operating the lathes were exposed to the hazard of unguarded rotating parts in that there were no chuck guards installed.
