29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Jobsite, Kenneth & Ridge Avenues, New Kensington, PA: On or about 09/27/12 employees working on a 10/12 pitch roof with unprotected sides and edges were not protected from falling approximately 18-24 feet to lower levels by guardrail systems, safety net systems or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, Kenneth & Ridge Avenues, New Kensington, PA: On or about 09/27/12 the employer had not provided a training program to employees potentially exposed to fall hazards to ensure the employee was able to recognize fall hazards and the procedures to minimize exposure to falling. Employees were working on a 10/12 pitch roof without fall protection, exposed to fall hazards of 18-24 feet, and the employees had not received any training from the employer on fall hazards and the procedures to protect themselves.
