29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: The employer does not ensure that employees are protected from electrical shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. This violation occurred on or about September 23, 2012, in the SW corner of the building where an employee was exposed to electrical hazards while accessing 277 VAC energized wiring. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from electrical shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.

This violation occurred on or about September 23, 2012, in the SW corner of the building where an employee was exposed to electrical hazards while accessing 277 VAC energized wiring from a 10 foot aluminum step ladder. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders used where the employee or the ladder could contact exposed energized electrical equipment have nonconductive siderails .

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident; The employer does not orally report within 8 hrs the death of an employee because of a work-related incident. This violation occurred on or about September 23, 2012 when an employee was electrocuted while installing wiring at or near 2580 FM 1960 in Houston, TX. The HNAO was made aware of the fatality from a media report on September 24, 2012. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employee work-related deaths are orally reported to OSHA within 8 hrs following the incident.
