29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    On or about October 3, 2012, Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    1. A 100-foot yellow electrical extension cord being used to power a Makita 2414NB 14-Inch Portable Cut-Off Saw was severely damaged, bare conductors were cut and exposed. The employee was exposed to electrical shock and/or electrocution.   2. The Makita 2414NB 14-Inch Portable Cut-Off Saw power had insulation damage. The employee was exposed to electrical shock.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems,  or personal fall arrest systems:    On or about October 3, 2012, an employee was observed working at the edge of a roof with an unprotected side or edge, which were 19-feet or more above a lower level. The employee was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems and was exposed to a fall hazard of approximately 19-feet to the ground.
