29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):      (a)  On or about 10/04/12 - Old Restaurant, the cover on a panel box was not installed exposing the live 480 volt wires.      (b) On or about 10/04/12 - Old Restaurant, the cover on a panel box was not installed exposing the live 220 volt wires.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:      (a)  On or about 10/04/12 - Catwalk above Casey's Bar, the cover on a 4x4 pull box was missing.
