This exposed employees to amputation hazards.

29 CFR 1910.136(a):     The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:    (a) The shaft on Winder 1 falls when employees are unloading a full spool and/or applying an empty spool. The company did not ensure that employees are wearing foot protection. This exposes employees to crushing hazards of their feet from falling objects.
