29 CFR 1910.22(d)(1):   In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate:    a)  On October 10, 2012, employees were exposed to being crushed by overhead materials, as there was an upper level floor in the warehouse of the main building, along the east wall, with no plates provided to display an approved load rating.     b)  On October 10, 2012,  employees were exposed to being crushed by overhead materials, as there was an upper level floor in the warehouse of the main building, along the north wall, with no plates provided to display an approved load rating.

29 CFR 1910.157(c)(1):   The employer did not provide portable fire extinguishers and mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury:    a)  On October 10, 2012, and at times prior thereto, employees were exposed to the hazards of a fire in the main retail store, as there was a "Fire Extinguisher" sign on the wall outside the northwest corner of the employees' breakroom, but there was no fire extinguisher in the area.

29 CFR 1910.157(e)(3):   The employer did not assure that portable fire extinguishers are subjected to an annual maintenance check:    On October 10, 2012, and at times prior thereto, employees were exposed to the hazards of a fire spreading in the south area of the main warehouse, as an Amerex nitrogen compressed fire extinguisher had an inspection tag showing it had not been inspected in two years.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that will prevent pull from being directly transmitted to joints or terminal screws:    On October 10, 2012, employees are exposed to electrical shock at the point the electrical cord of a Delta Industrial Table saw, Model Number 438-02-314-2032 attaches to the electrical box, not providing strain relief where it is worn, and wires are exposed.
