29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: An employee was using a full face negative pressure respirator. The employer did not have a respiratory program to ensure the employee had the proper training and was medically fit to wear the respirator. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On October 10, 2012 at a jobsite located at 919 Main Street, Naperville IL, employees were using a tubular welded scaffold to perform tuck pointing. The scaffold did not have a complete guardrail system, exposing employees a fall hazard of approximately 17.3 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

A hazard communication program should include but not limited to: 1. The purpose and scope of the program. 2. A list of hazardous chemicals being used. 3. Material Safety Data Sheets for all hazardous chemicals used. 4. Labeling and other forms of warnings. 5. Training agenda and requirements. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
