29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: (a) Middle of the Production Facility, Pallet Stacker - On or about October 10, 2012 employees were exposed to amputation hazards while working or traveling near a floor fan and they were not protected by a rear fan blade guard. The front fan blade guard had opening greater than 1/2 inch. (b) Maintenance Shop - On or about October 10, 2012 employees were exposed to amputation hazards from a floor fan blades. Employees were not protected by a guard with openings of 1/2 inch or less.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet: (a) Left Side of North Rollup Door - On or about October 10, 2012 employees were exposed to fire hazards while working or traveling near oxygen cylinders stored adjacent to fuel gas cylinders and they were not protected by a 20 foot separation between the oxygen and fuel gas cylinders.
