29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders' length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: (a) On or about October 9, 2012, Southern Colorado Roofing as the controlling employer, failed to ensure that employees of Mora's Roofing (subcontractor) were protected from falls while working on portable ladders. The portable ladders did not extend at least 3 feet above the upper landing surface being accessed.
