29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. At the site - An employee was exposed to an approximate 13'-8" fall hazard, in that, fall protection was not provided to an employee standing and riding on top of a pallet which was elevated by a rough terrain forklift. Violation observed on or about 10/11/12.
