29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Jobsite: The side rails of the aluminum extension ladder which was being used to access the roof did not extend at least 3 feet above the upper landing surface, for a jobsite located in Cumberland RI on or about 10-9-12.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Jobsite: Employees engaged in roofing work, at a height of 11 feet above the ground, were not protected by guardrail systems, safety net systems, or personal fall arrest systems, for a jobsite located in Cumberland RI on or about 10-9-12.  WB Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 426752, citation number 1, item number 1 and was affirmed as a final order on 6/29/12 with respect to a workplace located at 283-295 Newport Avenue in Pawtucket RI.
