29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)52 Rosehill, Irvington, NJ:Employee not wearing head protection with working being conducted directly above, exposing the employee to falling objects.         Violation observed on or about 10/13/12.    Payless Home Improvements, LLC, was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard which was contained in OSHA Inspection Number 316180934, Citation 2, Item 1 issued on 4/24/12 and a final order date of 6/19/12 with respect to a workplace located at 806 Ridge Road, Cedar Grove, NJ.

Violation observed on or about 10/13/12.
