29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    a)Computer Data Entry Room: On or about October 18. 2012 and at times prior, the employer exposed employees to electrical burns, and shock hazards, in that the 110 volt wall outlet was not covered exposing employees to electrical hazards while sitting at desk.

29 CFR 1910.141(d)(2)(ii): Lavatories were not provided with hot and cold, or tepid running water:    a) Unit A employee and visitor restroom: On or about October 18, 2012 and at times prior, the employer exposed employees to unsafe biological health hazard conditions in that there was no hot water source from the sink in the employee restroom and visitor restroom to be used to wash hands.
