29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    Jobsite:  The employees, stripping the existing asphalt shingles, were not protected from fall hazards by the use guardrails, safety nets or personal fall arrest systems.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Jobsite:  The employee who was exposed fall hazards of up to 19 feet 3 inches above the ground while working on 5 in 12 pitch roofs was not trained in fall hazards and the procedures to be followed to minimize such hazards.

29 CFR 1926.1053(b)(1):   Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    Jobsite:  The ladder that was used to gain access to the roof did not have its side rails extended 3 feet above the landing surface.
