29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) 28538 Dupont Blvd, Millsboro, DE - Employees observed working in an Vanguard (Baker) articulating aerial lift, approximately 15 - 20 feet above the ground, without fall protection. The employees used the lift to access the roof and remove debris. This exposed the employees to injuries from fall hazards. The violation was observed on or about October 11, 2012.

29 CFR 1926.1053(b)(1): Portable ladders used for access to an upper landing surface did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: a) 28538 Dupont Blvd, Millsboro, DE - Employees used a Louisville, 30 feet, aluminum extension ladder to access the roof, approximately 15-20 feet above the ground, without being extended 3 feet. The employees used the ladder to remove demolition materials from the roof. This exposed the employees to injuries from fall hazards. The violation was observed on or about October 11, 2012.
