29 CFR 1910.178(l)(4)(i): The powered industrial truck operators (PITs) were not trained and evaluted once in three years:    a) On or about October 17, 2012, at the facility, employees operating the Caterpillar propane gas fork lift had not been trained and evaluated since 2004. Employees used the fork lift to load the merchandise and to move materials in the shop. This condition exposed employees to struck-by hazards.
