OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from falling loads and/or lift parts: a) a) The employer does not furnish employment and a place of employment which is free from recognized hazards that causes or is likely to cause death or serious physical harm to employees. This violation occurred on or about October 16, 2012, when employees were using a snatch block with integrated hook (Gunnebo Johnson WR size 3/8 inch, WLL 4.4 Tons (4 MTons)) with a wire rope (Lift-All IWRC one-quarter (1/4) inch wire rope, 6x19) that was smaller than the size prescribed as acceptable by the manufacturer (three-eighths (3/8) inch minimum to (1/2) inch maximum diameter). b) The employer does not furnish employment and a place of employment which is free from recognized hazards that causes or is likely to cause death or serious physical harm to employees. This violation occurred on or about October 16, 2012, when employees were using a snatch block with integrated hook (Gunnebo Johnson WR size 3/8 inch, WLL 4.4 Tons (4 MTons)) with a base mounted drum hoist (My-Te AC-36 Base Mounted Drum Hoist; SN: E5680B67) where the fleet angle of three (3) degrees; greater than the generally accepted fleet angle for a smooth drum hoist of one and a half (1.5) degrees.

This violation occurred on or about October 16, 2012, when employees used wire rope rigging with broken wires, crushing, kinking, corrosion, and flattening that had not been inspected by a competent person before being used. b) The employer does not ensure that synthetic web slings, all fastenings and attachments are inspected for damage or defects by a competent person each day before being used, and during use, where service conditions warrant. This violation occurred on or about October 16, 2012, when employees used synthetic fiber slings with a snag and broken stitches that had not been inspected by a competent person before being used.
