29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Advanced Insurance Company - Employees were working on a steep roof 6 in 12 pitch installing roof shingles without fall protection such as: nominal 2 x 6 slide guards and/or conventional fall protection.  The highest point of the roof was about 15 feet above ground level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) The employer did not provide fall awareness or hazard recognition training pertaining to falls for employees conducting roofing activities on a steep roof (pitch 6 in 12) installing roof shingles 15 feet from the top of the roof and 10 feet from the eave without conventional fall protection. The employees had not received any training on fall hazards.
