29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) BP & Dunkin Donut - Employees were working inside a Skyjack SJ45T aerial lift truck approximately 12 feet high without a positioning device, which should have been attached, by lanyard, to either the basket or the boom of the lift, nailing wood board on the front of the door.  Employee was exposed to a fall hazard.
