29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:      (a) On or about October 19, 2012 - 20 Hazelwood Drive, Pell City, AL, an employee working from a Genie aerial lift using a nail gun to attach wooden eaves was not wearing eye protection.     (b) On or about October 19, 2012 - 20 Hazelwood Drive, Pell City, AL, an employee using a screw gun to attach Oriented Strand Board (OSB) to the metal frame of the building was not wearing eye protection.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:      (a) On or about October 19, 2012 - 20 Hazelwood Drive, Pell City, AL, an employee was working from a Genie aerial lift without the lanyard attached to the anchor point of the lift exposing the employee to fall hazards of up to 12 feet.
