A cylinder of LP gas was stored next to the loading dock and was exposed to physical damage from powered industrial vehicles and other moving equipment into the loading dock.

Employees conducting cleaning on metal frames with acetone were not provided with appropriate hand protection to prevent skin absorption when using the chemicals.
