(a)PPI, Inc. dba Prestique Exteriors, Inc. @ 22532 Indora Dr., Aurora, CO 80026: On and before October 1st, 2012, the controlling employer did not ensure that fall protection systems were utilized by subcontractor employees engaged in roofing activities while working at the unprotected edges of a roof on a single-family residential housing. Employees were exposed to fall hazards of approximately 21' on the primary roof and 10' on a secondary roof.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at  its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder    (a) PPI Inc. dba Prestique Exteriors, Inc. @ 22532 Indora Dr., Aurora, CO 80026:  On or before October 1st, 2012, the employer did not ensure that subcontractor employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  Subcontractor employees were using a portable extension ladder for access to the roof that did not extend 3' over the eave. Employees were exposed to a fall of approximately 10' while attempting to use the ladder.
