29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Residential site: On or about October 17, 2012 and at times prior thereto, 4 employees were exposed to an approximate 9 foot fall while tearing off the shingles on a single story home without fall protection or a safety monitor. Note: Abatement certification is required for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Residential site: On or about October 17, 2012 and at times prior thereto, 4 employees, exposed to an approximate 9 foot fall while tearing off the shingles on a single story home, were not properly and effectively trained to identify and mitigate hazards assoicated with working at elevations. Note: Abatement certification is required for this item.
