29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: JOB SITE: 7922 Dale Avenue, Richmond Heights, MO 63117, at this work site, workers were observed on the top the residential roof actively installing new roof shingles, using pneumatic nail guns without any form of eye or face protection. The workers were exposed to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems: or an alternative fall protection measure: JOB SITE: 7922 Dale Avenue, St. Louis, MO 62117, at this worksite, workers were observed on top of a 8:12 pitch residential roof, approximately 10 feet 1 inch above next lower level, actively installing new roof shingles, and were not protected by guardrail systems, safety net systems, personal fall arrest systems, nor were they using alternative fall protection measures and failed to develop and implement a satisfactory fall protection plan. The workers were exposed to a fall hazard.

The workers were exposed to a fall hazard.
