29 CFR 1926.452(c)(2):     Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.    a.  At the construction site located at 1293 Linwood Ave, Columbus, Ohio, employees were working replacing bricks on a chimney from an approximately twenty four foot tall scaffold that had cross bracing missing on the fourth level exposing them to a fall hazard.    b.  At the construction site located at 1293 Linwood Ave, Columbus, Ohio, employees were working replacing bricks on a chimney from an approximately twenty four foot tall scaffold that had cross bracing missing on the third level exposing them to a fall hazard.    c.  At the construction site located at 1293 Linwood Ave, Columbus, Ohio, employees were working replacing bricks on a chimney from an approximately twenty four foot tall scaffold that had a brace connection not secured on the first level exposing them to a fall hazard.

29 CFR 1926.452(c)(5)(iii):     Brackets used to support cantilevered loads shall:    Be used only to support personnel, unless the scaffold has been designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold.    a. At the construction site located at 1293 Linwood Ave, Columbus, Ohio, employees were installing brick and accessing the roof from a make shift wooden side bracket of a tubular welded frame scaffold that had wooden scaffold planks that extended perpendicular from the scaffold to the roof, which increased the tipping forces of the scaffold, thereby exposing the employees to a fall hazard of approximately 24 feet.

At the construction site located at 1293 Linwood Ave, Columbus, Ohio, employees were performing work on a roof replacing bricks on a chimney of a residential home with an approximately 5 in 12 pitch and approximately 20 feet in height from the ground to the eave with no fall protection.
