29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    Jobsite:  East side of building, north of main entrance    A subcontractor employee was exposed to a fall hazard of approximately 25 feet while working on a carpenter's bracket scaffold.  Just one 10 inch plank was used  on each section of the 36 inch wide scaffold brackets.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      Jobsite:  East side of building, north of main entrance    The carpenters bracket scaffold was accessed from a window opening.  The scaffold work platform was approximately 36 inches above the window sill.

Jobsite:  carpenter's bracket scaffold at east side of building, north of main entrance.    A subcontractor was exposed to a fall hazard of approximately 25 feet while working from a carpenters bracket scaffold.  The scaffold lacked guardrails and the employee did not use personal fall protection.

29 CFR 1926.1052(c)(1):     Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    Jobsite:  Main stairs between first and second floors.    The stairway lacked any handrails.  The walls on each side of the stairway was framed in.
